Passport Acceptance Facility

As a convenience to the community, Harford County Public Library System's Bel Air Library accepts passport applications on behalf of the U.S. Department of State. The Passport office is by appointment only. Please make your appointment below.

All of the information needed to process your application is listed on the application and can be found at, in the passport section. If you are missing anything at the time of processing, you will have to come back when you have all of the necessary documentation.

Appointments are scheduled for 20-minute increments. Appointments are limited to one applicant at a time. If you need multiple applications processed, please schedule an appointment for each, individual, applicant. All applicants must be present at the time of application, including children and infants.

If you are more than 5 minutes late to the appointment, and/or all persons required for the application process with the required paperwork are missing, you will forfeit your appointment and will have to reschedule for a later time.

Passport Rquirements

You must apply in person if any of the following is true

  • You have never been issued a U.S. passport or
  • You were under age 16 when your previous passport was issued or
  • Your most recent U.S. passport was issued more than 15 years ago or
  • Your most recent U.S. passport was lost or stolen or
  • Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change

Documents Required

  • The applicant, including infants and children, must be present.
  • Proof of citizenship - Example: an original birth certificate issued by the government (featuring an official seal, signature, and names of both parents). Hospital certificates and photocopies are not accepted); a Naturalization Certificate; a previous U.S. passport. A Permanent Resident Card is also required for children of Naturalized parent.
  • Current proof of identity - Example: valid driver's license/State ID/Government or Military ID.
  • Separate payment for DOS and HCPL fees -HCPL fees can be paid by Debit card or Credit card only. Department of State fees must be paid by check or money order only.
  • A completed, UNSIGNED application - Passport Application DS-11 must contain no cross-outs, white-out, or errors of any kind and must be completed in BLACK ink.
  • 1 color passport photograph, two-inches square, taken within the last six months, sharp focus with a white background. These may be taken at the library for $15.
  • Any previous U.S. passport book or U.S. passport card, unless it was lost or stolen. Please bring evidence in support of a name change, if applicable.
  • Two parents with proof of relationship for applicants age 0-15; or a parent with a notarized consent form DS-3053 and copy of both sides of non-appearing parent's ID. Proof includes birth certificates and adoption papers.
  • One parent for minors 16-17.

How to Apply

STEP 1: Complete and Print Application Form

Please visit the passport section at for the application. DS 11 must be filled out completely in black ink in order for your documents to be reviewed please do not sign the DS-11 form until the Passport Agent instructs you to do so.

Completed forms must contain no cross-outs, no white-out, or errors of any kind.

STEP 2: Submit Completed Form + Additional Documents

Submit your completed application form and the following additional documents.

  1. Proof of U.S. Citizenship: go to for all acceptable forms of citizenship.
  2. Proof of Identity: go to for all acceptable forms of identification.
    Note: The ID cannot be damaged and must readily identify you.
  3. Photograph: go to for details on the photograph. HCPL offers photo services during passport center hours for a fee (see below).

STEP 3: Pay the Applicable Fees

TWO payments are required: One made payable to the U.S. DEPT OF STATE and a credit or debit card payment for Harford County Public Library. The fee must be correct in order to be seen; see the State Department's website for more information about the correct fee schedule.
